Cannot display page after insert

I fill out my form and I click Insert a new record and it goes to Internet Explorer cannot display the webpage

Any ideas? My test site is http://www.quasndana.com

seems that I had to move the files to an actual directory and not the root folder of where my domain points. Any way to resolve this?

To use it at the domain root, make your site path like this:
$site_path = '/';
