Insert A New Record-error

Error during query execution.
Above is the page response on hitting "Insert a New record" I'm sure it's a simple fix I just can't fix it- All other componnents are working great!
running OSX 10.2.1
for internal logging only.
Any help would be apreciated!

Do you still have the problem?
Do you have blanks spaces in your field names?
